Matthew-Bender - TopForm Users

This information was supplied by Matthew-Bender. If you have questions about it, please contact marty.lemmond@lexisnexis.com or your designated Matthew-Bender technical support representative.   Attorney C.K. Belhasen has also prepared a generic temporary fix for EZ-File and other programs that return the "Office is a required field" error.

You can also add the attorney signature using TopForm.

Please note that Division codes that are necessary for electronic filing have not been included with TopForm. When you file a petition you will get an error:

"Office is a required field, An error occurred in BuildDebtorRec.”

 A. You may call the Topform Technical Support Hotline: 800-223-5297 for instructions on how to add the codes to your program. To save time, when you call them tell the representative that you need help adding office codes to the master database, that it is a known issue with the Eastern District of Kentucky, and that instructions for correcting the problem are already in the representative’s tech support database.

 B. Until you add the codes to Topform you can use the:

“Open a BK Case”

selection from the ECF menu instead of the “upload” option and manually enter the required information.

C. If you are an advanced computer user you may correct the problem yourself by applying the following steps. If you are not an advanced user, do not attempt to fix the problem by yourself, call TopForm / Lexis-Nexis technical support for assistance.

1. Before making any changes to any program it is a good idea to make a complete backup of the program so that you will not lose any data.

 2. Using Explorer, go to the directory that holds your TopForm files, usually C:\program files\topform or C:\topform

(Topform network version users will always have a master.bdb file on each workstation and on the server, and will sometimes have two master.bdb files on the server if it is also a workstation. To avoid confusion in picking the right file to correct, it is better to add the county codes to EVERY instance of master.bdb on each workstation and server.)

 3. Double click on the program DATAUTIL

 4. Click on “File”, then on the pull-down menu click on “Open Database”

 5. A browse window will open. Click on the folder “Master”, then click on the file “Master.dbd

 6. The database will open, with a list of tables on the left side of the window

 7. Scroll down and click on the “ County Codes ” table

 8. A table will open that has five columns (use the horizontal scroll bar to see all five). The columns / fields are “code” / “district” / “county” / “state” / with the last column being “office”

 9. Scroll down to the first rows that contain Kentucky counties. There are some 120 counties listed, 67 of which are in the Eastern District of Ky. They are in two groups – Anderson to Knox and Laurel to Woodford, separated by counties in other states (very roughly alphabetized by state). Slowly scroll through the rows and you will locate both groups.

 10. Using the following table VERY CAREFULLY enter the Division number (Ashland 1, Covington 2, Frankfort 3, Lexington 5, Corbin 6, Pikeville 7) in the Office column for each of the 67 counties in the Eastern District, OR at least for all counties that you may file petitions in.
