After evaluating comments received from the public in response to proposed revisions to Local Form 3015-1(a), Chapter 13 Plan, the court now publishes the final form which should be used for Chapter 13 cases filed on or after December 1, 2023 in the Eastern District of Kentucky Bankruptcy Court.
