OR - Completed Form 103B - Application to Have the Chapter 7 Filing Fee Waived
2. Original Signed Voluntary Petition
3. Completed Form 121 - Statement About Your Social Security Numbers
4. Completed Declaration Regarding Electronic Filing
NOTE: Each document must be signed with an original ink signature. Electronic or typed signatures (including /s/, DocuSign, or scanned images of signatures) are not sufficient.
